About Davita Forman, PA
Davita is board-certified in psychiatry and specializes in adults and adolescents suffering from mood disorders, psychotic disorders, substance use disorders, and dual diagnoses. Her goal is to help patients feel at ease. She utilizes medication management and motivational interviewing. Davita absolutely adores animals. She and her husband have a "Beatles"-themed menagerie of pets that include a dog named Lucy, a cat named Jude, two rabbits named Eleanor and Prudence, and a flock of quails known collectively as Sgt.
